Once you have made your appointment, you are welcome to pre-fill in your patient registration forms. See tabs in ‘Patient Information’ to find the ‘New Patient Form’ and ‘Medical Questionnaire’. Both forms are available to complete online or PDF forms are available to print and bring to reception at your first appointment.
We are a mixed billing practice. Your new patient appointment will only be bulk billed if you hold a current Health Care Card/ Pension Card/ Commonwealth Seniors card OR under 16 years of age. No concession entitlement will incur a fee on the day. Please see our ‘Fees’ tab located in ‘Patient Information’ for a full list of our fees. Medicare rebate is applied if your Medicare card is current.
Please also arrive at your appointment 15 minutes earlier for our friendly receptionists to complete your new patient registration.
